Change of Adviser/Major Form

  • Used when students change advisers, for example, at the end of the first year, and when declaring a major or academic interest. It is expected that students will declare a major no later than the end of their sophomore year.
  • Available from the Registrar's Office or online. Advisers should keep a supply in their office.
  • Requires signatures of the old and the new adviser, and the chairperson of the major department. The student is responsible for obtaining these signatures.
  • The student must take the completed form to the Registrar's Office. Failure to turn this form in to the Registrar means that no change in adviser/major will be recorded, and that the new adviser will not receive grade reports, etc.

Consortium Course Registration Form

  • Allows non-first year students to register at any of the seven Consortium Colleges for courses that are (1) not offered at Guilford College that semester, and (2) appropriate to the liberal arts program of Guilford College.
  • The Consortium Colleges are: Bennett College, Elon College, Greensboro College, Guilford Technical Community College, High Point University, N.C. A&T State University, and UNCG.
  • Forms may be obtained in the Registrar's Office. Class schedules for each of the Consortium colleges are available for perusal in the Registrar's Office, or on the Web page of some institutions.
  • All Consortium registrations are tentative subject to space available after the students from the host institution have registered.
  • After the form is filled out, and signed by the adviser, it must be approved and signed by the Registrar.
  • The Registrar will endorse and prepare the form for pick-up by the student on registration day. The student must take these copies to the Registrar of the host institution during their add period for the particular semester. Consortium forms will not be processed by the host school prior to their designated add period.
  • Be sure to process DropAdd slips through the Guilford Registrar for any section changes or closed Consortium courses.

Internship 290 or 390 Course Registration

Independent Study Course Registration

  • Independent Study courses carry either the 260, 360 or 460 course number.
  • May be designed for 1 to 4 credits.
  • No student may take more than two Independent Study courses, or 8 Independent Study credits per semester.
  • Independent Study forms are available in the Registrar's Office.
  • The student must receive approval of the instructor, the adviser, and the chair­person of the relevant depart­ment.
  • Completed forms must be taken by the student to the Registrar's Office no later than the close of the Add Period.

Transcript Requests

Transcripts can be ordered several ways

Online - By a secured Internet transaction over the World Wide Web, via credit card.

Log onto the national student clearinghouse.

If you are a first-time visitor, you will be directed to create a personal profile and user ID/password, which you will use to place future orders.

The site will walk you through placing your order, including delivery options and fees. You can order as many transcripts as you like in a single session.

Order updates will be emailed to you. You can also check order status or history online.

The cost for requesting transcripts online is $7.25, express shipping in the United States is an additional $25, Canada & Mexico is an additional $35 and International and additional $45. Faxed copies are an additional $5.

If you need the transcript to be issued to you in a sealed envelope with the Registrar's signature on the outside of the envelope, it must be specified in your written request.

NOTE: Telephone requests for transcripts are not accepted. An official transcript will not be issued if there is an outstanding financial or judicial obligation to the college.

For previously enrolled students, written requests should include full legal name, social security number, current address and telephone number, date of birth, dates of attendance, complete address where the transcript is to be sent, signature, and date of request.

For currently enrolled students, written requests should include name, student ID number, complete address where the transcript is to be sent, date of request, and signature.

Cost for each official transcript requested in advance is $10. The cost is $20 each if you are requesting us to fax a copy of your transcript somewhere. The cost for transcripts needed immediately (on demand) is $20 each. On demand transcripts are processed in the Registrar's Office Monday through Friday from 9:30AM. to 4:00PM. Payment may be made in cash, by check payable to Guilford College, or by credit card (American Express, Visa, Mastercard or Discover). Credit card payments must include American Express, Visa, Mastercard or Discover number, signature, Verification Code (See below for explanation), zip code of card owner and expiration date of card.

*The Security Code (CVV) for Visa, MasterCard and Discover cardholders is a 3-digit number found on the signature panel on the back of your credit card immediately after the card number or partial card number. For simplicity, the Security Code is always the last 3 digits shown on the signature panel.

The Security Code for American Express cardholders is 4 digits and is pre-printed on the front of the card above the account number. The Security Code is an additional safeguard that helps us validate your purchase and protect against fraud. It is not contained in the magnetic stripe information and is therefore not printed on sales receipts.

EDI transcripts are not currently available.

Faxed transcripts are available but are considered unofficial transcripts. A follow-up paper copy of a faxed transcript may be sent if an official transcript is needed.

Unofficial transcripts are available online using BannerWeb and are used for advisement purposes or for other informal uses.

Petition Forms for Academic Matters

Petition Forms are used to request an exception to normal academic procedures and regulations. The form requires a number of endorsements, including that of the Adviser. This request for an endorsement is designed to build a consensus favoring the requested exception!  Make your own determination as to the validity of the request before signing. Completed Petition Forms are returned to the Registrar's Office.

GRADE APPEALS are processed following the procedures given in the Student Handbook.
